Usuthu brought a completely different line-up after a semi-final win over Zululand District XI, as new signings Dumisani Zuma, Gabadinho Mhango and Augustine Kwem made way for fellow new recruits, the likes of George Maluleka and Rally Bwalya.
Brandon Truter's charges had a good start as Sandile Khumalo had a chance to give them an early lead at the back post after a set-piece delivery by Sphesihle Maduna but he was thwarted by goalkeeper Malcolm Jacobs.
Usuthu remained on the front foot with creativity, as the likes of new signing Thendo Makumela linked up play but they were not yet clicking in the last third.
However, AmaZulu could not maintain their intensity going forward in the latter phase of the first stanza while the Natal Rich Boyz did not trouble shot-stopper Veli Mothwa at the other end.
Usuthu continued to dictate terms from the half-time break as Lehlohonolo Majoro came close to breaking the deadlock on the hour-mark with a fine header that missed the target by inches.
Truter made a double change on 73 minutes as striker Bonginkosi Ntuli replaced Sbusiso Magaqa in a tactical switch while Majoro made way for Tebogo Masuku for fresh legs upfront.
However, AmaZulu and the Natal Rich Boyz did not threaten in the last third, forcing the final to be decided by a penalty shoot-out where Usuthu triumphed 4-2.
